Administrator / Director
Position: Administrator / Director (Assisted Living Facility)
Location: Fort Smith, AR
Qualifications: Experience in property management, personnel administration and provision of resident services. Maintains any certificates and educational requirements required by state regulation. Maintains CPR / First Aid

Duties and Responsibilities:
- Have knowledge of and comply with applicable laws and rules;
- Have knowledge and ability to deliver or direct the delivery of appropriate care to resident;
- Must meet the minimum education, training and experience requirements in one of the following ways:
- Successful completion of a program approved by the State of Arkansas; or
- Previous job related experience equivalent to successful completion of such program.
- Shall designate in writing a competent employee to act as administrator in his / her absence;
- Recruit, employ and train staff to meet the needs of the residents and facility;
- Maintain a 12-month record of daily work schedules or payroll records;
- Accept and retain only those residents who meet acceptance criteria and whose needs can be met by the facility;
- Maintain a log indicating any significant change in any resident's condition and the facility's action or response;
- Notify the resident's responsible person and health care provider of significant changes or deterioration of the resident's health and ensure his transfer to any appropriate health care facility if the resident requires services beyond the scope of the facility's license;
- Review every injury, accident and incident to a resident or employee, take appropriate corrective actions, and document corrective action taken;
- Notify the nearest peace officer, law enforcement agency, or a protective services agency whenever there is reason to believe that a resident has been subject to abuse, neglect or exploitation;
- Conduct regular inspections of the facility to ensure it is safe from potential hazards;
- Maintain any certificates and educational requirements;
- Provide orientation and inservices for the residents and staff;
- Maintain personnel records;
- Assure staff adherence to all policies and procedures;
- Assess residents and develop a Negotiated Service Plan, implement a
- Managed Risk Agreement as necessary;
- Schedule and assist residents with counsels and act as an advocate;
- Maintain resident service quality;
- Assure all financial and administrative records are maintained;
- Maintain marketing activities to assure full occupancy;
- Maintain staffing to meet 24 hour, seven days a week general monitoring of the residents;
- Maintain an on-call schedule;
- Perform other duties as directed.
Position: Personal Care Attendant
Qualifications: Has certification as required by state regulation. Maintains CPR/First Aid,
Reports to: Administrator
Duties and Responsibilities:
1. Adhere to policies and procedures.
2. Assist resident with ADL's and environmental orientation as needed.
3. Assist with preparation of the dining room and serving of the meals.
4. Assist with housekeeping and laundry services.
5. Observe residents and report any variances to the administrator or licensed staff.
6. Provide all services according to the Service Plan.
7. Demonstrate knowledge of standard precautions and infection control.
8. Demonstrate knowledge of and adheres to procedure for fire, life safety, disaster, work safety and other emergency procedures.
9. Assist with activities.
10. Complete personal care including baths, back rubs, oral hygiene, shampoos, and to change bed linens as often as necessary.
11. Assist in dressing and undressing residents.
12. Provide proper care and observation of resident's skin to prevent breakdown of tissue over bony prominences.
13. Report on resident's condition and significant changes to the Administrator and Health and Wellness Director
14. Perform other duties as assigned.
Position: Cook
Qualifications: Maintains any certifications or permits required by regulations.
Reports to: Administrator
Duties and Responsibilities:
1. Follow menu cycle and use of production guides.
2. Prepare meals according to schedule.
3. Maintain a clean, safe working environment and food sanitation guidelines.
4. Work with the residents to meet their preferences and needs.
5. Maintain food service reports according to regulation.
6. Demonstrate knowledge and adheres to procedures for fire, life safety, disaster, security and other emergency procedures.
7. Maintain inventory control.
8. Prepare, receives and verify orders.
9. Maintain inventory to meet the needs of regulation.
10. Perform other duties as assigned.
11. Maintain CPR and First Aid Certification.
